Abstract
Pasaulyje heterozigotine šeimine hipercholesterolemija (HeŠH) serga nuo 14 iki 34 milijonų gyventojų t. y. 1 iš 500, tačiau daugelyje šalių ši liga diagnozuojama tik 1 proc. gyventojų [1]. Sergantiesiems šeimine hipercholesterolemija (ŠH) nuo gimimo yra padidėjęs mažo tankio cholesterolio (MTL-C) kiekis kraujyje, todėl tokiems pacientams vainikinių arterijų ligos (VAL) rizika yra 13 kartų didesnė nei sveikų gyventojų [2]. HeŠH sergantys pacientai VAL suserga 20 metų anksčiau nei nesergantys gyventojai, t. y. apie penkiasdešimtuosius gyvenimo metus. Jei šeimoje yra HeŠH sergantis asmuo, tikslinga atlikti jo pirmos eilės giminaičių kaskadinę patikrą. Nustačius HeŠH, tikslinga kuo anksčiau pradėti nemedikamentinį ir medikamentinį gydymą. Šiuo metu ŠH diagnozuojama per vėlai – dažniausiai po pirmojo ūminio išeminio sindromo, todėl tikslinga sukurti ir įdiegti veiksmingą gyventojų patikros programą, padedančią anksti diagnozuoti HeŠH, kad būtų galima kuo anksčiau pradėti gydyti ir išvengti ankstyvosios VAL bei kardialinės mirties.
Between 14 and 34 million people in the world are affected by heterozygous familial hypercholesterolaemia (HeFH), 1 of 500 people have this disease, yet only 1 perc. of the affected are diagnosed [1]. People affected by this disease have an increased low-density lipoprotein (LDL) concentration in the blood, thus the risk of coronary heart disease (CHD) is increased 13 times [2]. The onset of CHD in patients with HeFH is 20 years earlier compared to the general population and usually occures around the age of 50. If HeFH is diagnosed, a cascade screening of patient's family members should be performed. When diagnosed, it is important to start the treatment with medication and lifestyle corrections as soon as possible. Nowadays, familial hypercholesterolaemia is diagnosed too late, usually after the first onset of acute coronary syndrome (ACS). This prompts the need for a functioning diagnostic program to help diagnose patients with HeFH and start the treatment to avoid ACS and cardial death.
