Abstract
Nepaisant veiksmingų vaistų, optimalių gydymo rekomendacijų, nekontroliuojamos arterinės hipertenzijos (AH) dažnis nekinta, o 1996–2006 m. jos sąlygotas mirtingumas padidėjo 19,5 proc. Viena šios tendencijos priežasčių – nenuosekliai laikomasi mokslinių rekomendacijų. Tyrimo tikslas. Išanalizavus Lietuvoje išrašytų receptų duomenis, įvertinti sergančiuosius AH medikamentinio gydymo pasirinkimą. Nuo 2012 m. lapkričio 21 d. iki 2012 m. lapkričio 30 d. AH gydyti išrašyta 89 119 receptų: 35 proc. pacientų išrašytas vienas vaistas, 41 proc. – du, 18 proc. – trys, 6 proc. – daugiau nei trys vaistai. Dažniausiai išrašyti: beta adrenoblokatoriai (BAB), angiotenziną konvertuojančio fermento inhibitoriai (AKFI), angiotenzino receptorių blokatoriai (ARB), kalcio kanalų blokatoriai (KKB), centrinio veikimo ir kitos vaistų klasės. Du vaistai išrašyti 37 009 pacientams. Dažniausi dviejų vaistų deriniai – AKFI ir BAB, AKFI ir diuretiko, AKFI ir KKB. Dažniausi neoptimalūs dviejų vaistų deriniai – AKFI ir BAB (6811 receptų, 18,4 proc.), BAB ir centrinio veikimo vaistai (3 proc.). Išvados. AH gydymo schemos praktiniame darbe neatitinka ekspertų parengtų rekomendacijų. Gydytojas, prieš skirdamas vaistus sergančiajam AH, turėtų įvertinti gretutines ligas, kitų ligų riziką ir tada skirti optimalų gydymą.
Despite the eff ective drugs and optimal therapy recommendations the incidence of uncontrolled hypertension remains unchanged and from 1996 till 2006 mortality caused by arterial hypertension increased by 19.5%. One of the possible explanations of these trends – the imbalance between scientifi c guidelines and the daily clinical practice. Th e aim – to assess arterial hypertension pharmacological treatment options in Lithuania analyzing the data of diff erent drug classes prescription. From November 21th, 2012 till November 30th, 2012 there were 89,119 prescriptions for patients with arterial hypertension: for 35% of patients one drug was prescribed, 41% – two drugs, 18% – three drugs, 6% more than three drugs. Th e frequency of prescription of drugs to treat hypertension were as follows: beta-blockers (BB), angiotensin-converting enzyme inhibitors (ACE), angiotensin receptor blockers (ARB), calcium channel blockers (CCB), centrally acting drugs and other drug classes. Two were prescribed for 37.009 patients. Th e most commonly prescribed two drug combinations were ACE and BB, ACE and diuretic, ACE and CCB combination. Lithuanian prescriptions’ data suggests that a non-optimal two-drug combination treatment of essential hypertension – ACE and BB combination is oft en prescribed (6811 prescriptions, 18.4%). Another inappropriate combination of two drugs – BB, and centrally acting drugs – were prescribed to 3% of patients. Conclusion. Arterial hypertension treatment schemes in the practical work oft en do not match with experts recommendations. Every doctor while treating a patient with arterial hypertension prior to prescribing medication should assess pat ient’s comorbidities, risk of other diseases and provide optimal individualized antihypertensive treatment.
