Abstract
The aim of the study: to determine prognostic possibilities of C-reactive protein and hematocrit in predicting the course of acute pancreatitis. Materials and methods: the multicenter prospective study was conducted at four hospitals in Vilnius and Kaunas. Blood serum tests for patients with acute pancreatitis were carried out on the 1st and 3rd day their hospitalization in order to determine the CRP and hematocrit values. According to the revision of the Atlanta consensus, patients were divided into three groups based on the course of the disease: mild acute pancreatitis, moderate severity pancreatitis and severe acute pancreatitis. CRP and hematocrit values were compared among groups and, by drawing ROC curves, cut-off values for the prediction of severe acute pancreatitis were determined. Results: The study included 102 patients with acute pancreatitis: 27 (26,5%) of them were diagnosed with mild acute pancreatitis, 55 (53,9%) - with moderate severity pancreatitis and 20 (19,6%) - with severe pancreatitis. It has been determined that the CRP cut-off value which according to statistics is reliable in recognizing severe pancreatitis from mild acute and moderate severity disease on the third day of hospitalization is 301,1 mg/l (sensitivity 70,7%, specificity 80,0%, AUC 0,79). On the first day of hospitalization for the recognition of severe acute pancreatitis the 44.3% hematocrit value is reliable (sensitivity 68,3%, specificity 85,0%, AUC 0,77). [...].
