Abstract
Introduction Cholangiocarcinoma (CCA) is the second most common primary liver malignancy. According to the anatomical location CCA is classified to the intrahepatic (iCCA) and extrahepatic (eCCA), the latter is subdivided into perihilar (pCCA) and distal (dCCA). CCA incidence and mortality rates are increasing worldwide. Radical surgery is the only curative treatment for CCA, however, most CCAs are diagnosed in the advanced stages. The prognosis of patients with CCA is usually poor, with a median survival of less than 24 months after diagnosis. Aim To investigate the age of onset, types of CCA, treatment and survival rate in patients with CCA. Methods Patients diagnosed with CCA hospitalized in the Lithuanian University of Health Sciences Hospital between 2012 and 2018 were included in this single-center retrospective study. Patient and treatment data was collected from hospital records. Mann–Whitney U-test and Kruskal-Wallis tests were used to compare continuous variables. Results 26 patients were included in the study: 17 (65.4%) women. Median age at the time of diagnosis was 65.5 years (range 37-99), with no significant difference between genders (p=0.107). ICCA was diagnosed in 11 (42.3%) patients, while 15 (57.7%) patients were diagnosed with eCCA: 11 (73.3%) pCCA, 4 (26.7%) dCCA. 4 patients (15.4 %) underwent radical surgery as the initial treatment, Chemotherapy was applied to 10 (38.5%) patients and 12 (46.2 %) patients received best supportive care. Median survival rate was 5 months (range 0-60). Patients, who received best supportive care, had lower survival rate (1.5 months (range 0-18)) than those who were treated systemically (5 months (range 2-20) or surgically (26.5 months (range 0-60) (p=0.03).
