Abstract
Introduction Accelerated corneal cross-linking (A-CXL) procedure aims to halt the progression of keratoconus. Nonetheless, the long-term effects of A-CXL on post-operative corneal transparency are relatively unknown. Our aim was to assess the 18-month changes in corneal densitometry values after A-CXL in patients with progressive keratoconus (PKC). Method In this retrospective study 16 eyes of 15 patients with PKC, who received A-CXL from 2016 to 2018 at the Hospital of LUHS Kaunas Clinics Department of Ophthalmology (Kaunas, Lithuania), were enrolled. Parameters of corneal densitometry (CD) were evaluated at the baseline and 3, 6, 12 and 18 months after A-CXL using Pentacam ®. CD was measured in total thickness (TT) and anterior, central and posterior layers of the corneal stroma in 4 annular diameters: 0-2 mm (∅0-2mm), ∅2-6mm, ∅6-10mm and ∅10-12mm. The statistical analysis was performed using IBM SPSS v. 25.0 software. Results There was a significant increase of the mean CD values of the TT over ∅0-2mm, ∅2-6 mm at third(23.49±3.79 and 19.01±2.38, respectively), sixth(20.06±2.27 and 16.35±1.26, respectively) postoperative month(PM) and ∅0-2mm at 12(18.17±3.34), 18(18.52±2.53) PM in relation to baseline results(P<0.05), in the anterior and central layers over ∅0-2mm, ∅2-6 mm at 3, 6 and 12 PM and over ∅0-2mm at 18PM(P<0.05). The mean CD values of the posterior layer were significantly higher only at 3PM over ∅0-2mm(9.84±2.54), ∅2-6 mm(10.53±2.26) in respect of baseline results(7.47±1.29 and 8.56±0.87, respectively). We found no statistically significant changes in any of the layers of corneal stroma over ∅6-10mm and ∅10-12mm after A-CXL. Conclusion The values of corneal densitometry remain increased over the postoperative period of 18 months after A-CXL.
