Abstract
Darbo tikslas. Įvertinti Kauno miesto stacionaruose dirbančių akušerių nuomonę apie jų savarankiškumą darbe bei sąsajas su socialiniais ir demografiniais veiksniais. Uždaviniai. 1. Įvertinti akušerių nuomonę apie jų savarankiškumo darbe galimybes bei sąsajas su socialiniais ir demografiniais veiksniais. 2. Įvertinti akušerių nuomonę apie jų savarankiškumo darbe poreikį bei sąsajas su socialiniais ir demografiniais veiksniais. 3. Įvertinti akušerių nuomonę apie jų savarankiškumo darbe trukdžius bei sąsajas su socialiniais ir demografiniais veiksniais. Metodika. Atlikta Kauno miesto stacionaruose dirbančių akušerių anoniminė anketinė apklausa, apklausiant visas 146 (atsako dažnis 100 proc.) akušeres. Tyrimas buvo vykdomas 2019 gegužės 13 d. - 2019 birželio 29 d. Duomenų analizė buvo atlikta naudojantis SPSS 23.0. Dėl mažos imties, dviejų požymių nepriklausomumo sąsajos buvo tikrinamos naudojantis Fisher exact kriterijumi, dviejų nepriklausomų imčių proporcijų lygybė buvo tikrinama Z kriterijumi. Požymių skirtumai laikyti statistiškai reikšmingais, kai p<0,05. Rezultatai. 60,9 proc. Kauno miesto gimdymo stacionaruose apklaustų akušerių nurodė, jog dirba savarankiškai. Nustatyta, kad savarankiškai dažniau dirba tos akušerės, kurios turi trumpesnį darbo stažo laiką bei yra jaunesnės nei 40 m. amžiaus, nei tos, kurių darbo stažas ilgesnis ir yra vyresnės. Akušerės, kurios dirba rotuojantis skyriuose, teigia, kad rečiau dirba savarankiškai, o akušerės, dirbančios gimdymo arba akušerijos skyriuose, dažniau savarankiškai priima sprendimus bei atlieka veiksmus/procedūras pacientėms. Savarankiškai dirbančios akušerės, turinčios ≤20 m. darbo stažą (69,9 proc.), dirbančios rotuojant per skyrius (91,2 proc.) nurodė, jog savarankiškumo jų darbe nėra (p<0,001). Nustatyta, kad ≥41 m. amžiaus grupėje 19,2 proc. respondenčių visada priima sprendimus savarankiškai. Akušerių savarankiškumo darbe trukdžius vertino tik tos respondentės, kurios nurodė, jog darbe nėra savarankiškos. Dažniausiai akušerių savarankiškumo darbe trūksta, nes: su akušerių savarankiškumo didėjimu nesutinka gydytojai (p<0,01), akušerės nenori prisiimti tokios didelės atsakomybės (p<0,001), jos nepasitiki savo jėgomis (p<0,001) arba nurodo patirties trūkumo veiksnį (p<0,01). Išvados. 1. Daugiau nei pusė apklaustų Kauno miesto stacionaruose dirbančių akušerių teigia, jog įstaigoje, kurioje dirba, nėščiąsias ir gimdyves prižiūri savarankiškai. 2. Daugiausiai akušerės jau iki šiol savarankiškai atliko veiksmus/procedūras bei priėmė sprendimus dėl nėščiųjų ir gimdyvių priežiūros. Vyresnės nei 40 metų amžiaus respondentės, lyginant su jaunesnėmis respondentėmis, dažniau nurodė, jog joms trūksta savarankiškumo priimant sprendimus ar atliekant veiksmus/procedūras, todėl jos labai norėtų turėti daugiau savarankiškumo. 3. Vienas pagrindinių trukdžių, neleidžiantis akušerėms savarankiškai atlikti veiksmų/procedūrų bei priimti sprendimų yra akušerių patirties trūkumas bei nenoras prisiimti atsakomybės.
Aim of the study. To analyze the opinion of midwifes about their independence at work in Kaunas hospitals and connections with social and demographic factors. Objectives. 1. To evaluate the opinion of midwifes about the possibilities of their independence at work and connections with social and demographic factors. 2. To assess the opinion of midwifes about the need for their independence at work and connections with social and demographic factors. 3. To evaluate the opinion of midwifes about the obstacles to their independence at work and connections with social and demographic factors. Methods. An anonymous questionnaire survey of midwifes working in Kaunas city hospitals was conducted, interviewing 146 (response rate 100%) midwives. The study was conducted from May 13, 2019 to June 29, 2019. Data analysis was performed using SPSS 23.0. Due to a small sample, associations between two variables were checked using the Fisher exact criterion, and the equality of the proportions of the two independent samples was checked using the Z criterion. Differences between variables were considered statistically significant when p<0.05. Results. 60.9% of the interviewed midwifes in Kaunas city maternity hospitals indicated that they work independently. It was found that midwifes who have a shorter length of work experience and are younger than 40 years are more likely to work independently than those with longer work experience and older in age. Midwifes who work rotating in hospital wards say they are less likely to be self-independent, while midwifes who work in maternity or midwifery department wards are more likely to make decisions independently and perform actions / procedures for patients. Independently working midwifes with ≤20 m. length of work experience (69,9%) who work rotating through departments (91.2%) indicated that there is no independence in their work (p<0.001). It was found that in ≥41 years of age group 19.2% of respondents always make decisions independently. Obstacles to midwifes‘ independence at work were assessed only by those respondents who indicated that they were not independent at work. Usually midwifes lack independence at work because: doctors do not agree with the increase in midwifes‘ independence (p<0.01), midwifes do not want to take such a great responsibility (p<0.001), they do not trust in their own strength (p<0.001) or indicate a factor of lacking experience (p<0.01). Conclusions. 1. More than half of the interviewed midwifes working in Kaunas city hospitals state that they take care of pregnant women and mothers independently in the institutions they work. 2. Most midwifes have already performed actions / procedures independently and made decisions on the care of pregnant women and mothers. Respondents over the age of 40, in comparison with the younger respondents, more often reported the lack of autonomy in decision-making or actions / procedures, and therefore would be more willing to have more autonomy. 3. One of the main obstacles preventing midwifes from performing actions / procedures and making decisions on their own is the lack of experience and reluctance to take responsibility.
