Abstract
Tyrimo tikslas buvo ištirti ultrastruktūrinius enterinės nervų sistemos pokyčius, kurie vyksta sergant divertikulioze.
Ištirta 25 pacientų storosios žarnos operacinė medžiaga. 16 pacientų buvo operuoti dėl storosios žarnos vėžio. 10 pacientų neturėjo divertikulų ir buvo naudoti kaip kontrolinė grupė, 6 pacientai turėjo divertikulų, bet iki šiol neturėjo virškinimo trakto nusiskundimų, ši medžiaga naudota kaip besimptomė grupė. 9 pacientai buvo elektyviai operuoti dėl pasikartojančio divertikulito priepuolių, jų medžiaga buvo naudota kaip divertikuliozės grupė. Operacinės medžiagos mėginiai buvo padalinti į tris dalis, jog kiekvienoje iš dalių atitinkamai būtų mienterinis, išorinis pogleivio ir vidinis pogleivio rezginiai. Medžiaga buvo fiksuota ir dažyta rutiniškai transmisinei elektroninei mikroskopijai. Gautose nuotraukose buvo vertinami kokybiniai enterinės nervų sistemos pokyčiai, nervinių ataugų pažeidimas ir jų dydis.
Sergant divertikulioze neuronuose buvo rasta daugiau į lipofusciną panašios medžiagos, membraninių organelių cisternos buvo padidėjusios ir buvo rasta mažiau ryšių su aplinkinėmis nervinės ataugomis. Dėl divertikuliozės visuose nerviniuose rezginiuose stebėta daugiau pažeistų, formą praradusių nervinių ataugų ir daugiau smulkių nervinių ataugų. Vidiniame pogleivio rezginyje sergant divertikulioze rasta daugiau putliųjų ląstelių, turinčių kontaktą nervinėmis skaidulomis ir intersticinėmis Cajal ląstelėmis. Dalis intersticinių Cajal ląstelių sergant divertikulioze tarpusavyje išlaikė ryšius, tačiau prarado kontaktą enterine nervų sistema ir lygiosiomis raumenų ląstelėmis.
Divertikuliozės pažeistuose neuronuose stebimi degeneraciniai pokyčiai. Dėl divertikuliozės visuose nerviniuose rezginiuose stebima nervinių ataugų remodeliacija, pasireiškianti didesniu degeneruojančių ir smulkių nervinių ataugų kiekiu, taip pat vidiniame pogleiviniame rezginyje aplinkui nervus rasta daugiau putliųjų ląstelių. Intersticinės Cajal ląstelės neteko ryšių su aplinkinėmis ląstelėmis.
The aim of the study was to investigate the ultrastructural changes of the enteric nervous system in diverticular disease.
Specimens were collected from 25 patients. 16 patients were operated for colorectal cancer. 10 patients did not have macroscopic diverticula and were used as a control group, 6 patients had macroscopic diverticula and no gastrointestinal symptoms related to diverticula and were used as a asymptomatic diverticulosis group. 9 patients underwent an elective surgery after recurrent attack of diverticulitis and were used as a diverticular disease group. The specimens were divided into three parts, each containing the myenteric, the outer submucosal and the inner submucosal plexuses. Tissue samples were fixed and stained for transmission electron microscopy. Qualitative changes in the enteric nervous system, the size and degeneration of neuronal processes were evaluated in the electronograms.
In diverticular disease, neurons exhibited larger lipofuscin-like inclusions, membranous organelles had larger cisterns and there were less contacts with the surroundings nerve processes. Nerve remodelling occured in every plexus. There were more degenerating nerve processes as well as more fine nerve fibres. In the inner submucosal plexus, mast cells impinged on nerve fibres and interstitial cells of Cajal. Some of the interstitial cells of Cajal maintained contacts with each other, however, contact with smooth muscle cells and nerve fibres was compromised due to increased collagen fibre deposition between interstitial cells and the surrounding tissue.
In diverticular disease, neurons showed signs of degeneration. In all the enteric plexuses, remodelling occured increased numbers of degenerating and fine nerve processes. Mast cells were found near nerve fibres of the inner submucosal plexus in diverticular diease affected tissue. Interstitial Cells of Cajal lost contacts with the surrounding cells.
